WORKPLACE SELF-ASSESSMENT
Find out if performance design is right for you
What this assessment measures
Environmental performance risk is the degree to which a physical workspace restricts the effectiveness of the people working within it.
As part of its full-scale performance design work, Life Form carries out a site observation, which assesses sixteen environmental elements grouped into six performance impact categories, including Focus & Cognitive Load, Energy & Activation, Regulation & Recovery, Spatial Effectiveness, Social Cohesion & Culture, and Meaning & Motivation. Each category’s underlying elements are assessed and scored from 0 (meaning no impact) to 4 (meaning critical impact).
Life Form’s workplace self-assessment — a streamlined version of Life Form's comprehensive site observation — reviews high-level performance risk categories that a business leader can observe unaided, with the aim of providing an indication of environmental performance risk within their workplace.
How the rating is read
Optimised (0–20% impact risk)
Isolated Risks (21–40% impact risk)
Moderate Risks (41–60% impact risk)
High Risk (61–80% impact risk)
Critical (81–100% impact risk)
What a self-assessment can’t tell you
Roughly half of the sixteen elements Life Form assesses are observable without instruments. These are addressed within this workplace self-assessment.
The remaining elements require measurement against defined thresholds. Examples include CO₂ concentration in parts per million under closed-door meeting conditions, lux levels and colour temperature by zone and by hour, humidity and particulate matter, temperature variance across a floor, and more. None of these can be eyeballed, and several have been the highest-risk findings in workplaces that scored well on everything visible.
The workplace self-assessment is an indicative screen only. It points at where risk is likely concentrated; it cannot tell you magnitude, cause, or impact, nor can it definitively recommend individual initiatives or environmental enhancements to improve performance.
